How Much BAC Water to Add to 3mg Semaglutide: Concentration Guide

There is no universal answer to how much bacteriostatic (BAC) water to add to a 3mg semaglutide vial; the correct amount depends on the concentration specified by the dispensing pharmacy. From a purely mathematical perspective, adding 1mL of bacteriostatic water to a vial containing 3mg of semaglutide produces a concentration of 3mg/mL, while adding 2mL produces a more diluted concentration of 1.5mg/mL. Neither calculation establishes which volume is appropriate for a particular product.
FDA-approved injectable semaglutide products, including Ozempic and Wegovy, are supplied as ready-to-use liquid solutions in prefilled pens and should not be reconstituted with bacteriostatic water. A 3mg powder vial is therefore likely to be a compounded or unapproved product. Only use the diluent, concentration and instructions provided by a licensed prescriber and dispensing pharmacy.
Adding 0.5mL of BAC water yields a 6mg/mL solution (0.60mg per 0.1mL), 1mL yields 3mg/mL (0.30mg per 0.1mL), 1.5mL yields 2mg/mL (0.20mg per 0.1mL), 2mL yields 1.5mg/mL (0.15mg per 0.1mL), 2.5mL yields 1.2mg/mL (0.12mg per 0.1mL), and 3mL yields 1mg/mL (0.10mg per 0.1mL). On a U-100 insulin syringe, 0.1mL corresponds to 10 units.
These calculations assume the vial contains exactly 3mg, the full stated amount dissolves, the final liquid volume equals the amount of BAC water added, a correctly calibrated U-100 syringe is used, and there is no measurable loss during preparation. Real compounded products may contain excipients or use a different final volume, so the pharmacy label must take priority over a general calculator.
Adding more water does not increase or decrease the nominal amount of semaglutide in the vial; it changes how much semaglutide is contained in each millilitre. The concentration can be calculated with the formula: Concentration = semaglutide in vial ÷ liquid volume. Because different compounded semaglutide products can contain different concentrations, the same syringe measurement could deliver substantially different amounts. For example, someone using 10 units from a 6mg/mL solution would receive six times as much semaglutide as someone using 10 units from a 1mg/mL solution.
The FDA has received reports of adverse events—including cases requiring hospitalisation—associated with compounded semaglutide dosing errors. Some patients administered five to 20 times the intended amount after confusing milligrams, millilitres and syringe units.
