Ovarian Cancer Pipeline Advances: BriaCell, MIRASOL Trial, and OVACell

BriaCell Therapeutics Corp. (Nasdaq: BCTX, BCTXL) (TSX: BCT) announced progress in developing Bria-OVA+, a next-generation, personalized, off-the-shelf, cell-based immunotherapy for ovarian cancer, while new trial data for the targeted therapy mirvetuximab soravtansine showed survival and quality-of-life benefits in platinum-resistant ovarian cancer. A third initiative, the European OVACell project, is advancing T cell therapies that target viral sequences abnormally expressed in tumor cells.

Bria-OVA+ is designed with additional immune-stimulating components to support enhanced anti-tumor activity. BriaCell said the expansion builds on encouraging efficacy and tolerability data from its Phase 2 study in metastatic breast cancer and reflects its strategy to build on the Bria-OTS+ platform and become a leader in women's health. According to the company's President and CEO, Bria-OVA+ reflects a broader strategy to advance next-generation personalized immunotherapies for difficult-to-treat cancers. The Chief Medical Officer said ovarian cancer remains the deadliest gynecologic cancer and patients who do not respond to available therapies face serious unmet medical need.

BriaCell recently reported preclinical data for Bria-BRES+, its next-generation breast cancer immunotherapy candidate derived from the same Bria-OTS+ platform. In a recent AACR poster presentation, Bria-BRES+ demonstrated activation of both adaptive and innate immunity, including naïve/resting T cells, dendritic cells, and natural killer (NK) cells. BriaCell has licensed ovarian cancer cell lines from American Type Culture Collection (ATCC) and has commenced development activities to support production of Bria-OVA+ for potential clinical use.

The Phase III MIRASOL trial enrolled more than 450 women with folate receptor-alpha (FRa)-positive, platinum-resistant ovarian cancer and compared mirvetuximab soravtansine, marketed as Elahere, with standard chemotherapy. Results showed:

• Longer overall survival: median survival approximately 16.5 months versus about 12.8 months with chemotherapy.
• Longer progression-free survival: 5.6 months versus roughly 4 months.
• Higher tumor response rates.
• Reduced risk of death by about one-third.

Patient-reported outcomes showed better quality of life and fewer severe treatment-related side effects compared with chemotherapy. Common side effects include nausea, fatigue, blurred vision, and eye-related complications such as keratopathy, requiring regular eye examinations. Health authorities in several countries, including England's National Health Service, have approved access to the therapy for eligible patients.

The OVACell project, led by ErVimmune, is developing T cell therapies that target unconventional antigens derived from human endogenous retroviruses (HERVs), viral sequences normally dormant in human DNA but abnormally expressed in tumor cells. Founded in 2020 by a physician and academic oncologist, ErVimmune first developed a vaccine for triple-negative breast cancer and ovarian cancer targeting sequences shared across Europe, North America, and ASEAN regions, and is now advancing a T cell therapy for ovarian cancer. The project uses a proprietary methodology including AI-driven data analysis, immunopeptidomics, and in vitro testing. With support from the European Innovation Council, OVACell is preparing for preclinical evaluation, with the goal of demonstrating safety and efficacy in humans in the coming years.

According to the National Cancer Institute, an estimated 21,010 women in the U.S. will be diagnosed with ovarian cancer and approximately 12,450 will die from the disease in 2026. Ovarian cancer remains the deadliest gynecologic cancer. Existing treatments include surgery, platinum-based chemotherapy, Elahere, and the PARP inhibitor Lynparza, though many patients do not respond to these treatments.

Following the BriaCell announcement, BCTX declined 14.81% to $3.46, removing approximately $4 million from the company's valuation, with trading volume at 2.8 times the average.
